Because some youth activiities take place off of the church campus we want to make sure that you have been thouroghly informed as to where your youth will be when they are with us. That is why we have ask you to sign a permission slip for your youth to be under our supervision while off the church campus.
Second, because we know that some church activities like camping, rafting, hiking, biking, etc have some risk of injury involved, we are asking you to put a permenant permission slip on file with the church, including your medical insurance information so that, if the need arises, we can get timely emergency care for your child. This permission slip and insurance card will go with us when we are out of town. We will always seek to to reach you in the event of an emergency so that you can make decisions about medical care, however, we all want to make sure that your child can receive timely care if necessary.
Once you have filled out the Medical release form -- it will be good for the whole year. Thank you for your cooperation.
